Definition of Care and Service

The Admission Nurse/CM must have a significant role in evaluating patients, and formally admitting or filling out file paperwork. The Admission Nurse/CM will engage in clinical assessments of patients. Record tracking may also be involved, and a liaison will help form future processes for dealing with patient data. Scope of service will include:

• Work with all clinical teams and groups in evaluating both current and potential patients' statuses and medical needs
• Serves as an educational resource for facility employees and family members
• Complete all reports required by the facility
• Reviewed all patients referred for admissions and assist with the patient transition period
• Display a professional customer service attitude toward patient and family members
• Demonstrates Clinical and Service excellence behavior in daily interactions with patients, families, co-workers and physicians.
• Other duties as assigned if no incoming admissions on the shift.

Supervisor: Admission Supervisor/Case Management

Hours: M- F 12pm-8:30pm or 11:30am-8:00pm

Qualifications: Must have a current license as an LPN or RN

Experience: 1-2 years acute or subacute care. Marketing experience a plus.

Competencies:

The Admissions Nurse will demonstrate:

• Complete required admission documents with patient/families. 
• Respond to referral sources in a timely manner; evaluate patient appropriateness for skilled nursing.
• Meet with families/patients to discuss any concerns upon admissions into the facility 
• Serves as a representative and member of marketing/admissions team
• Communicate and provide daily feedback to admission staff on referral
